In some instances, additional medical records are required prior to issuance of a Medical Examiners Certificate, as per FMCSA Regulations. To better prepare for your appointment, we have listed that information below. Please contact out office with any questions about these conditions or any other medical conditions or medication use.

MEDICAL RECORDS TO BRING TO OFFICE

SLEEP APNEA:

Please bring a CPAP Compliance Report for the last 90 days of use. This is issued by the company that provided your CPAP. They may be able to read the data remotely and email it directly to my office. With older machines, you may have to have the data card read at the equipment supplier.   

CARDIAC CONDITIONS:

Any history of heart surgery, heart attack, bypass, stents, pacemaker - you must provide a copy of the LVEF% from your cardiologist. This information is obtained from an echocardiogram or exercise tolerance test (stress test). This number must be 40% or greater.

NON-INSULIN DEPENDENT DIABETES MELLITUS:

Please bring the most recent A1C% Report from your primary care doctor. This must be below 8%, as required by the FMCSA. 

INSULIN-DEPENDENT DIABETES MELLITUS:

MONOCULAR VISION:

This report is required to provide information for an individual who has “monocular vision,” as defined by FMCSA, or did not meet FMCSA's vision standard at a physical qualification examination.
